The Vice President of Population Health is responsible for leading and executing strategies that improve population health outcomes while ensuring financial and operational success in value-based care models. This executive role oversees the Accountable Care Organization (ACO) and Population Health initiatives, working collaboratively with internal and external stakeholders to drive quality, efficiency, and patient-centered care.

Responsibilities

The Vice President of Population Health will assist in payer relations, specifically with Community Care of Oklahoma (CCOK), ensuring strong partnerships that align with the organization's value-based care goals. This position requires a strategic and data-driven leader with expertise in healthcare transformation, quality improvement, and innovative care delivery models.

Leadership & Strategy:

  • Develop and lead the strategic vision for population health and ACO operations, ensuring alignment with organizational priorities.
  • Direct and oversee quality improvement initiatives, care coordination models, and population health programs.
  • Provide executive oversight to the Quality & Population Health Director and Director of Value-Based Programs, ensuring strong operational execution and continuous improvement.
  • Serve as a key leader in payer relations, collaborating with CCOK and other payers to enhance value-based care partnerships and reimbursement models.
  • Align population health strategies between the ACO and the employed provider group, physician leadership, operational teams, and external stakeholders to optimize performance in risk-based contracts.

ACO & Population Health Oversight:

  • Provide leadership and oversight for the Accountable Care Organization (ACO), ensuring operational excellence and regulatory compliance.
  • Implement data-driven strategies to enhance care coordination, chronic disease management, and preventive care efforts.
  • Oversee population health analytics and track key performance indicators (KPIs), including quality measures, utilization trends, and financial performance.
    Ensure compliance with CMS, Medicare Shared Savings Program (MSSP), and other value-based care regulations.
  • Collaborate with clinical and operational teams to drive patient engagement and reduce healthcare costs through effective population health initiatives.

Payer Relations & Value-Based Care Partnerships:

  • Establish and foster strong payer relationships, particularly with Community Care of Oklahoma (CCOK), to enhance value-based agreements.
  • Lead contract negotiations for alternative payment models (APMs), shared savings arrangements, and risk-based contracts.
  • Ensure alignment between payer initiatives and clinical workflows, working closely with medical directors and operational teams.
  • Identify and implement strategic initiatives to optimize reimbursement, reduce financial risk, and improve patient outcomes.

Operational & Performance Improvement:

  • Partner with clinical leadership, care management teams, and analytics departments to enhance provider engagement and care delivery.
  • Oversee care management workflows, social determinants of health (SDOH) initiatives, and patient engagement strategies.
  • Drive the development and adoption of evidence-based care pathways and protocols across the network.
  • Lead cross-functional teams in identifying and scaling best practices across all clinics and provider groups.

Direct Reports & Team Leadership: responsible for guiding these teams to ensure alignment with the organization’s overall objectives.

  • Provide executive oversight and mentorship to the following direct reports; will be responsible for guiding these teams to ensure alignment with the organization’s overall objectives:
    • Quality & Population Health Director – responsible for managing clinical quality initiatives, value based contracts and populations for employed providers, population health analytics.
    • Director of Value-Based Programs – responsible for management of ACO including financial performance, and provider engagement strategies.
    • Establish clear performance goals and accountability measures for direct reports, ensuring alignment with organizational priorities.
    • Support team development and ensure the adoption of best practices in population health and value-based care.

Skills & Competencies:

  • Strategic thinker with a proven ability to lead large-scale healthcare transformation efforts.
  • Strong financial acumen with experience managing value-based contracts, reimbursement models, and shared savings programs.Excellent communication and relationship-building skills, especially in provider engagement and payer negotiations.
    • Proficiency in using healthcare analytics and population health platforms to drive data-informed decisions.
    • Proven success in payer relations, including negotiating value-based contracts and risk-sharing arrangements.
    • Deep knowledge of ACO operations, CMS regulations, quality metrics (HEDIS, Stars, etc.), and healthcare data analytics.Experience managing multi-disciplinary teams and collaborating with provider networks.
  • Minimum 5-7 years of leadership experience in population health, ACO management, or value-based care strategy.
  • Master’s degree preferred in Healthcare Administration, Public Health, Business Administration, or a related field required.

Requirements

Education:

  • Associate's degree/Bachelor's degree with 7 years of applicable cumulative job specific experience required, with 4 of those years being in leadership/management. OR 7 years of experience required.
  • 10 years of experience preferred.
  • 5 years of leadership or management experience preferred.
