This involves a deep understanding of value-based care, specifically full-risk / capitated arrangements, along with the managed care levers available to drive superior clinical and financial results. This Senior Medical Director role will put you at the leading edge of the value-based care movement by being a part of our Medical Management leadership team.The Senior Medical Director of Transitional Care will be responsible for co-leading, with operational leaders, a range of clinical initiatives related to improve transitions of care for some of our most vulnerable patients. The Senior Medical Director of Transitional Care will provide clinical leadership to a portfolio of programs within this domain and may also supervise other Clinical Leaders performing this work. This is a fast-paced, high visibility role to ensure that we are driving innovation and improvement rapidly and consistently within Oak Street Health and CVS Healthspire’s Risk Bearing Entity.This role reports up through the Healthcare Delivery organization at CVS Health and will work closely with leaders across Oak Street Health, the Risk Bearing Entity, and the broader CVS enterprise.Core Responsibilities:In addition to clinical duties caring for Oak Street patients as a provider 2 days per week in one of our Centers or home-based programs, the Senior Medical Director is responsible for the following general leadership and administrative duties (note an addendum may be added to this job description based on the specific focus areas for a given open role):- Organizational Collaboration: In partnership with other senior Medical Management leadership, the Senior Medical Director for Transitional Care will help lead development for predefined pillars of the Oak Street Health care model that relate to coordination for patients transitioning from acute to post-acute and home levels of care in order to prevent unnecessary hospital readmissions. This will include helping to determine tactics to achieve organizational goals surrounding readmission rate and other indicators of successful transitions of care, vetting new internal and external partnership opportunities, and creating clinical content. The Senior Medical Director will also collaborate closely with Population Health, Central Services, and Divisional / Regional leaders in these efforts. Additionally, the Senior Medical Director may be asked to participate or lead workgroups or committees.
- Transitional Care program clinical leadership: This physician or Advanced Practice Provider will be the clinical leader for a set of programs and/or initiatives within the Transitional Care and Centralized Services Teams. Most often, the Senior Medical Director will be partnered with an operational leader for each program. As the clinical leader for the programs, it will be the Senior Medical Director responsibility to assure the clinical services are high quality, patient centered and address Oak Street Health and CVS Healthspire's broader goals. Depending on the program design, the Senior Medical Director may be expected to provide direct clinical supervision to RNs, social workers, APPs, physicians, PharmDs, and/or other clinical or support staff involved in delivering care through these programs.
- Lead Collaboration with clinical and operational leaders: The Senior Medical Director will lead bidirectional communications with stakeholders across the organization related to their programs and initiatives. The Senior Medical Director will be expected to lead the gathering of input from the Medical Directors of our centers, regions and divisions and other programs, as well as keep leaders across the organization informed of our transitional care model planning and programmatic efforts.
- Supervise and develop Clinical Leaders: Within the Medical Management leadership teams, the Senior Medical Director may supervise and/or support Medical Directors and Clinical Directors & Managers performing programmatic work to enhance the impact of their work and develop their communication, and operations skills. This could additionally include clinical supervision of practicing clinicians and clinical leaders assigned to contribute subject matter expertise to guidelines, clinical decision support, informatics, or other efforts.

- US work authorization
- M.D./ D.O./APN / PA clinical training including an active license to practice. Physicians should be Board certified in Internal Medicine, Family Medicine, Preventative Medicine or Geriatrics.
- Fellowship training, professional degrees (e.g., M.B.A., J.D., M.P.H.), and/or 2-5 years of management consulting experience preferred.
- Training and experience providing primary care is preferred.
- 1-3 years of clinical leadership or supervision experience is required.
- Experience with managed care and/or value-based practice and familiarity with payer-provider collaboration.
- Experience using a metrics-driven approach to analyze cost, quality, and satisfaction data to drive clinical strategy and program redesign.
- Experience developing and implementing new processes.
- Understand the basics of managed care

The typical pay range for this role is:
$184,112.00 - $396,550.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
