Title: Nurse Preceptor

Role Description

The purpose of a Nurse (RN) Preceptor is to educate and empower our Center Nurses to deliver high-quality patient care by equipping them with the knowledge and skills they need.

At Oak Street Health, you will serve as a dedicated preceptor for new hires and support skill-building for established Oaky Nurses through the delivery of local training for onboarding, new company-wide or regional implementation support, and remediation for skills and competencies. This role requires high levels of flexibility, energy, attention to detail, mastery of the role, skill in training, and problem-solving skills. You will be expected to build relationships with a wide variety of Oak Street Health team members (Oakies) throughout the region and across the organization, working frequently with Oaky Nurses throughout their lifecycle and collaborating closely with Directors of Clinical Support and center-level leaders to meet identified needs for precepting throughout the region(s) in which you cover. You will work to create an engaging and welcoming environment through team communication and delegation to empower other members of the care team to deliver the best care to our patients.

During non-precepting hours, RN Preceptors may provide float RN coverage supporting center-level needs throughout the region, assist in driving quality care improvements in alignment with regional priorities, and perform other duties to support regional performance as needed.

RN Preceptors report to the regional Director of Clinical Support.

Core Responsibilities

Nurse Onboarding for New Oaky Nurses (0.5 - 0.6 FTE, or more as needed in region)

  • Displaying proficiency in this role and executing job responsibilities in a safe and consistent manner

  • Create a welcoming and engaging environment to meet the needs of new Oaky Nurses

  • Demonstrate excellence in interpersonal skills through adaptable communication techniques and eagerness for collaboration

  • Provide competent nursing education by demonstrating sound clinical knowledge and clinical excellence

  • Developing skills and competencies needed to be an effective preceptor by attending required training and skill development sessions.

  • Execute local onboarding of all new Oaky Nurses

  • Help a new Oaky put into practice what they have learned in onboarding and assess readiness to perform independently in role

  • Empower RN IIs, who serve as center-level site mentors

  • Support remediation and performance coaching for Oaky nurses

Nurse Training and Reinforcement for current RNs (0.2 - 0.3 FTE - secondary to Precepting responsibilities for new hires)

  • Actively collaborate and monitor the outcomes of training initiatives and implementations

  • Identification of opportunities for quality care improvements through chart audits, observation, and feedback

  • Execute training experiences to support company-wide and regional initiatives/implementations

  • Support remediation and performance coaching for Oaky nurses

Float Coverage (0.1 - 0.2 FTE - tertiary to Precepting and Training responsibilities)

  • Provide competent nursing care by displaying proficiency in this role and executing job responsibilities in a safe and consistent manner

  • Respond to incoming telephonic requests in a dependable manner, ensuring we are responsive to their needs and exceeding expectations

  • Provide clinically competent triage and symptom management to patients who may or may not be physically present

  • Utilize standardized protocols for medication management, prescription refills and prior authorizations.

  • Conduct thorough and accurate reviews of patient medications and update as needed

  • Provide comprehensive education and direct patient care, particularly around chronic conditions; may occur in person, over the phone or in group settings

  • Actively collaborate and monitor the implementation and progress of the care plan for patients on multiple provider panels

  • Form relationships with patients and their caregivers to support preventative care and ED/hospital diversion where appropriate

  • Create a welcoming and engaging environment to meet the needs of our patients, communities, families and teams where they are

  • Delegation of activities to other clinical care team members to support the needs of our patients

  • Participate in care team meetings to discuss patient care and clinic operations

  • Deliver an exceptional patient experience through service, responsiveness and respectful care

  • Perform point of care testing, procedures and specimen collection (including phlebotomy) as needed

  • Performs other related duties as assigned






What we’re looking for

Required Qualifications:

  • Active Registered Nurse (RN) Licensure in good standing with the applicable state

  • Successful mastery of the workflow in their previous RN II position

  • 1-year experience as a Nurse at Oak Street Health

  • BLS Certification

  • Electronic Medical Record (EMR) experience

  • Ability to maintain patient confidentiality and process information in a confidential manner

  • US work authorization

  • Ability to assess patients without face-to-face interaction, strong communication and assessment skill

  • Candidate must be willing to travel to all centers regionally to support precepting and have access to a reliable form of transportation

Strongly Preferred Qualifications:

  • 3+ years of healthcare experience working as an RN

  • Successful mastery of the workflow in their previous RN II position

  • Ability to collaborate and communicate with members of an interdisciplinary care team

  • Ability to work independently

  • Excellent computer skills with the ability to read, interpret and analyze data from various computer systems

  • Effective problem-solving and prioritization skills

  • Previous experience in clinic setting

  • Successful mastery of the work required of the RN II position

  • Commitment and passion for training and continuing education

  • An appetite for expanded responsibilities, greater clinical experience, and a chance to truly make an impact in the growth and development of their Oaky teammates

Preferred Qualifications:

  • Fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities we serve (where necessary - suggestion to add this as an optional data point for specific demographics, removing from our generic central posting)

Pay Range

The typical pay range for this role is:

$54,095.00 - $116,760.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
