Position Summary:
Utilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote/influence members in decisions related to achieving and maintaining optimal nutritional status.
Assessment of Members:
- Through the use of clinical tools and information/data review, uses knowledge and experience of nutrition and diet planning to conduct comprehensive evaluation of member's nutritional status and risks.
- Utilizes assessment techniques to determine member’s level of health literacy, technology capabilities/access, and readiness to change.
Enhancement of Medical Appropriateness and Quality of Care:
- Assimilate health/nutritional assessment data to establish a recommended nutritional approach based on member’s health status/risks, education, preferences, and support needs
- Partners with members to identify opportunities for integration into available internal/external programs.
- Using holistic approach consults with supervisors or others to overcome barriers with assisting members in meeting goals and objectives.
- Identifies and escalates quality of care issues through established channels.
- Utilizes influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum level of health.
- Utilizing interviewing and active listening skills, provides nutritional coaching, information and support to empower the member to make ongoing independent healthy lifestyle choices.
- Interprets and utilizes clinical guidelines/criteria to positively impact members health.
-Provides up-to-date information to help facilitate the member’s understanding of his/her health status.
-Helps member actively and knowledgably participate with their provider in healthcare decision-making.
Essential Functions:
- Experience in disease management and provision of Medical Nutrition Therapy
- Develop and monitor established nutrition plans in collaboration with the member to meet the member’s goals
- Demonstrate the ability to communicate, problem solve, work effectively with people, and use good judgement in challenging situations
-Utilize interviewing and active listening skills, provide nutritional coaching, information, and support to empower members to make ongoing independent healthy lifestyle choices
-Ability to influence/motivate members and promote positive behavior change through a mix of communication mediums
-strong organization and time management skills
Required Qualifications:
- Minimum of 5 years of experience working in a multidisciplinary team
- Minimum of 5 years of experience with weight management, disease management, medical nutrition therapy and health lifestyle behavior change
- Minimum of 5 years of coaching experience
- Registered Dietitian through the Commission on Dietetic Registration required
- Registered Dietician Licensure required in the state of residence if applicable
- Upon hire, it will be required to obtain licensure in multiple states where members reside, with CVS supporting the cost of required licensure
Preferred Qualifications
-Certified Diabetes Care and Education Specialist (CDCES)
-Health Coach certification
- Experience in facilitating patient/client positive behavioral change.
- preferred.
- Managed Care Organization experience preferred
- Exceptional computer skills including MS Office Suites to include Outlook, Word and Excel required.
- Demonstrated ability to communicate, problem solve, and work effectively with people, use good judgment in challenging situations
-Candidates in Mountain or Pacific time zones preferred
Education
- Bachelors required
- Registered Dietitian through the Commission on Dietetic Registration

The typical pay range for this role is:
$43,888.00 - $111,351.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
