Job Location GRAND Addiction Recovery Center - Tulsa, OK Position Type Full Time Salary Range $20.40 Hourly

Description

Responsibilities:

  1. Assesses client needs by completing a Needs Assessment
    1. Creates a client Care Plan based on Needs Assessment to ensure client needs are addressed and appropriate referral, linkage, and advocacy is provided
  2. Works closely with counselor/rehab educator/PRSS on identified needs, makes referrals, arranges transportation, and provides follow-up to assure identified needs are met
  3. Conducts client follow-up as appropriate
  4. Documents in client record services provided and follow up appointments from outside agencies
  5. Provides education and support to clients and facilitates groups for clients when assigned and/or serves as backup
  6. Meets with treatment teams as scheduled to review progress and status of clients
  7. Works with clients to assure appropriate plans are in place in preparation for discharge, including but not limited to linking clients to appointments/facilities for continued treatment after discharge
  8. Faxes discharge paperwork in required timeframe to payor source or other identified party when appropriate and requested
  9. Communicates with payor sources on a regular and continued basis throughout client’s treatment
  10. Will provide follow-up calls or letters to clients if they miss any scheduled appointments
  11. Meets with caseload at least monthly
  12. Completes reports weekly and as requested
  13. Performs other duties as assigned

Qualifications

Education and Experience:

ODMHSAS Case Management 1 certification required.

Previous experience working in a substance abuse treatment setting preferred.

Knowledge and Skills:

Knowledge of recovery programs and the disease concept of chemical dependency and co-occurring disorders, confidentiality laws, and community resources.

Ability to recognize signs and symptoms of withdrawal from alcohol and other drugs; communicate with clients as well as co-workers; document clearly in the client record; appropriately and accurately assess client needs; provide crisis intervention and maintain control; work and coordinate with outside agencies.
