Consistent performance has helped the hospital be nationally recognized for patient safety. </li></ul> Responsibilities <ul> <li>Interpret orders and transcribe or verify them accurately and efficiently in the pharmacy computer system.</li> <li>Accurately compound and dispense pharmaceuticals.</li> <li>Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.</li> <li>Provide accurate and timely medication information to healthcare staff and patients.</li> <li>Effectively and proactively interact with medical staff regarding drug therapy issue</li> <li>Document all clinical activities and interventions accurately and completely.</li> <li>Sustain the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and other approved formulary protocols and/or guardrails.</li> <li>Supervise and assist pharmacy technicians and other support staff effectively.</li> <li>Train and participate with Disaster Team drills and real-event response as assigned.</li> <li>Prioritize work and lead others to comply with facility medication turn-around time policy and procedure and identify areas for improved efficiency.</li> <li>Actively participate in staff meetings and attends staff development in pharmaceutical care to identify learning and performance needs.</li> <li>Support in the training and mentorship of new staff members as assigned.</li> <li>Must serve as preceptor for pharmacy students and/or residents.</li> <li>Support and participate in the goals of the organization’s quality improvement, risk management, and patient safety program to include but not limited to National Patient Safety Goals, Core Measures, and HCAHPS medication related initiatives.</li> <li>Maintain professional competency sufficient to meet current and future needs of the facility.</li> <li>Maintain appropriate controlled substance records.</li> <li>Comply with all regulatory standards.</li> <li>Make appropriate evidence-based, patient-centered medication recommendations.</li> <li>Participate in the management of medical emergencies as appropriate, including, but not limited to Code Blue.</li> <li>Provide clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.</li> <li>Work with the Pharmacy Director or Operations Manager to identify and develop processes to meet or exceed all regulatory standards and participate in quality and regulatory reviews and audits.</li> </ul> Qualifications Job Requirements <ul> <li>B.S. or Pharm D. from an ACPE-accredited School of Pharmacy.</li> <li>Active Pharmacist license in good standing with the Oklahoma State Board of Pharmacy.</li> <li>BLS &amp; ACLS certification (BLS required within 90 days of hire; ACLS required for attending Code Blue).</li> </ul> Preferred Job Requirements <ul> <li>Doctor of Pharmacy from an ACPE-accredited college of pharmacy.</li> <li>1 year inpatient hospital pharmacy experience.</li> </ul>
