This includes patient assessment and education on diabetes self-management</li> <li>Provides education and training for care managers, medical assistants and nursing staff regarding diabetes self-management using the 3 M of self-care and other care plans/resources in alignment with ADA guidelines. </li> <li>Assess clients’ nutritional, health needs and social determinant needs </li> <li>Conduct in person, Televideo and groups education and counseling </li> <li>Participates in community events on the topic of healthy eating and diabetes education. </li> <li>Documentation care plans in Epic</li> <li>Evaluate if the meal plans have a positive effect on clients’ health</li> <li>Create educational material about healthy eating habits</li> <li>Speak to groups about the importance of nutrition to prevent specific diseases</li> <li>Document patient progress</li> </ul> Qualifications Job Requirements: <ul> <li>Certification as a Diabetes Educator by the American Diabetes Association</li> <li> Registered dietitian in the state of Oklahoma OR Registered nurse in the state of Oklahoma </li> </ul> Preferred Job Requirements: <ul><li>3 years of experience as a dietitian or RN</li></ul>
